Sutter Heart & Vascular Institute specializes in providing comprehensive and compassionate care to patients with heart failure. The Sutter team will work collaboratively with you from diagnosis to long-term living.

Your treatment will be multi-disciplinary, with your primary care physician, cardiologists, nurse coordinators, telemanagement care nurses, social workers, and dieticians providing specialized care at a recognized center of excellence in heart care. If appropriate, you will also be screened for transplantation as well. Sutter Heart & Vascular Institute provides specialized care, including External Counterpulsation and Cardiac Rehabilitation.

Your relationship to your primary care physician is important, but the Sutter Heart & Vascular Institute team is here to help too. Our team of specialists will be contact with your doctor regarding your progress and any changes in your condition.

The Sutter Heart Failure Clinic assists patients with heart failure (HF) through regular interaction. When you participate in the program, specially trained staff will provide consultation, support, and information by making regular contact to you. This way, you can remain in the comfort of your home while staying up-to-date on your condition and treatment programs.

Your physician, the telemanagement nurses, and you will work collaboratively to provide the best care for your condition. Sometimes, referrals to other professional health care staff may be appropriate, such as cardiac rehabilitation specialists, dietitians, and other providers.
