    SeniorCare PACE
    Sutter Health Sacramento Sierra Region

Sutter SeniorCare (SSC) is a non-profit health plan exclusively for frail older adults. SSC’s model of care is known as the “Program of All-Inclusive Care for the Elderly” or PACE. The PACE model has been recognized as the most appropriate model of comprehensive care for the frail elderly population.

For more information on PACE programs, visit www.pace4your.org.

SSC currently serves a 34-zip code area and has two sites. The mission of SSC is to:

    • Provide high quality care and services to its participants
