    Family Medicine Residency Program

    PGY1 applications for the 2012-13 academic year will be accepted only through ERAS beginning September 1, 2011 through December 1, 2011. Applications received after December 1 will be considered on a case by case basis. All applications are screened based on merit.

    To be considered an application will include:

    • Common Application form
    • Personal Statement
    • USMLE or COMLEX Step I Score
    • Official Transcript
    • Three Letters of Recommendation

    In some instances a decision may not be made until the following is also included in the application:

    • Dean’s Letter

    Applications from International Medical Graduates must also include the following before being considered:

    • Post Graduate Training Authorization Letter with No Restrictions (PTAL)
    • ECFMG Certification

    Please note, International Medical Graduates may apply without the PTAL or ECFMG certification to meet the December 1 deadline; however, the applicant will not be considered for interview until these documents are part of the application. If you are applying with visa requirements we will only accept a J-1 visa.

    Please complete the additional information below along with your first three choices for availability to interview and submit to our residency office by hitting the “Send to Residency Program” button below.
