Medical Records
There are a couple of different options to request medical records:
- A patient can send or fax a letter to the hospital requesting copies of his or her records. The letter must contain the dates of the hospital stay(s) requested, full name and birth date and must be dated and signed by the patient.
- The patient can come in to the Medical Records department and fill out a request for records.
If the records are being requested by another health care provider the patient can sign the release form supplied by the provider and the provider will send the request to the hospital medical records department. The Release of Information staff will send copies via mail or fax to the provider office at no charge to the patient.
Patient medical records requests are completed within 15 business days of receiving the request and the charge for copies is $.25 per page.
All Release of Information offices listed below are open Monday through Friday 8:00 a.m. to 4:30 p.m. (closed on holidays).
