WE CARE provides medical case management to women in Sacramento County diagnosed with HIV/AIDS. WE CARE reaches out to women who are newly diagnosed, high risk, or outside of treatment. The goal of WE CARE is to eliminate barriers to medical and self care and assist women as they move into a formal care system.

The program has provided HIV testing and counseling to more than 2,300 high-risk women and provided intensive medical case management and home visits to more than half of the women diagnosed with HIV/AIDS. Of the many pregnant women (some diagnosed during pregnancy), the program has helped ensure no HIV transmission to their babies.

Funding for the highly respected program is provided by the Sacramento County, Ryan White Care Act Fund, and Sutter Medical Center Foundation.
